Specialty Pharmacy Providers: Partners for Improving Care
Specialty pharmacy providers (SPPs) play a central role in the distribution of APOKYN® to your patients.
An SPP can offer your patients the following benefits:
- Individualize the APOKYN shipment schedule for the patient
- Provide all the ancillary supplies needed for APOKYN administration, including needles, alcohol swabs, and a "sharps" container
The APOKYN SPPs also offer patient counseling, education, and training services through a toll-free helpline staffed by qualified healthcare professionals.
Working with SPPs
The following SPPs currently offer APOKYN:
- Accredo Nova Factor, Memphis, Tenn; 1-866-898-0104 (phone); 1-866-898-0069 (fax)
- PharmaCare, Pittsburgh, Pa; 1-800-238-7828 (phone); 1-877-287-7226 (fax)
You may select an SPP to deliver APOKYN to your patient. In some cases, the patient's insurance company will determine which SPP ultimately provides service.
Enrolling a Patient With an SPP
Once you have established the correct dose of APOKYN for a patient, simply complete an APOKYN Prescription & Enrollment Form or VA Enrollment Form for the patient. Fax the completed form to the SPP you have selected:
- Accredo Nova Factor, 1-866-898-0104 (phone); 1-866-898-0069 (fax)
- PharmaCare, 1-800-238-7828 (phone); 1-877-287-7226 (fax)
