The APOKYN Circle of Care™
The APOKYN Circle of Care is a unique program designed to assist patients and care partners in the use of APOKYN® for the treatment of off-episode motor symptoms associated with advancing Parkinson's disease.

APOKYN Call Center (Option 2)
The APOKYN Call Center is a convenient source of information about APOKYN and the use of the APOKYN pen.
Through the APOKYN Call Center, you can speak with specially trained nurses, get answers to frequently asked questions about APOKYN, and receive direct access to additional services.
APOKYN Call Center services are offered Monday through Friday, 8 AM to 8 PM Eastern Time. After-hours and weekend voice mail and return call service are available.

Home Healthcare Visits (Option 3)
One-on-one Home Healthcare visits by specially trained nurses are provided. Home Healthcare is available at no cost to all APOKYN patients whose healthcare professionals request the service for them.
If requested by a doctor, a Home Healthcare nurse will visit the patient's home a few days after the initiation of APOKYN. The nurse will help ensure that the APOKYN patient is receiving the dose of APOKYN recommended by the healthcare professional.
After each Home Healthcare visit, the nurse will complete a patient report to update the healthcare professional.

APOKYN Circle of Care Member Support (Option 2)
To receive additional support with your treatment, please sign up to become a member of the APOKYN Circle of Care. You can do so by calling 1‑877‑7APOKYN (1‑877‑727‑6596).
Program members receive:
- Follow-up phone calls from APOKYN Call Center nurses who will:
- Address any questions and concerns that you may have
- Provide support to help you with APOKYN therapy
- Information and materials designed to:
- Help you get the most from APOKYN therapy
- Help you manage Parkinson's disease

| 90% of patients achieved a therapeutic response within 20 minutes that was approximately equivalent to their usual response to levodopa (n=20). APOKYN is used as needed to treat off-episode motor symptoms, including muscle stiffness, slow movements, and difficulty starting movements, associated with advanced PD. |
